Fields not visible

I want to show all the fields also when somebody is logged in. Now I can't put in info like name, category or something else and that is generating an error when you want to submit the ticket. Please see the attachment, no fill in or select options so not possible to work with...

So I need to have all the fill in options en select options visible. I can't find out how to do that so I can't work with Helpdesk Pro anymore..

Look like there is some css conflict with your site template causing the error. Please submit a support ticket sending us super admin account of your site and link to the page. Our template developer will check to see what's wrong and get it sorted
