At the moment custom fields can only be allocated to published plans.
This makes it impossible to set up a bunch of plans (that need custom fields) in advance and then just make them live with a single click (each).
Please put all (unexpired, perhaps?) plans in the "plans" dropdown in the custom field creation/editing dialogue
