Plan with multiple fee options

Hi there,

Is it possible to have a plan (same form) including this:
1. Different fees options: one by default and 3/4 radio buttons with other options (see screenshot for example)
2. A text-area field in which the costumer can introduce an amount (greater than a given number, which would be default fee)
