As replied via support ticket, this issue happens because you are having few forms on one page.

At the moment, we only support one form per page only

It's not working for me also.
I only have 1 form and it's not adding the custom field.
You have a problem in the code, it adds the "Values" instead of the "Fee Values". Please check it out, I have done testing.

This feature has been working fine for years, so I believe it is just something wrong with your setup somehow

Could you please submit a support ticket sending us super admin account of your site and link to the form so that I can check it?
