Donation Campaign with unique url

Donation Campaign with unique url was created by Drew M
Ability to have unique url for each campaign. Donation page will not have drop down to choose campaign, but campaign_id will appear in hidden field.
For Example:
index.php?option=com_jdonation&view=jdonation&cid=1
Campaign description can appear at top of form.
For Example:
index.php?option=com_jdonation&view=jdonation&cid=1
Campaign description can appear at top of form.
