Error when submitting donation

Hi Tuan,
I'm having some trouble with the donation form. After I fill out all the information and try to submit the donation, it just takes me a blank screen and nothing happens. I've tried uninstalling and reinstalling the component and it still doesn't work. Please advise! Thanks!
