MemPro signup breaks the template

Hi

I am using the combination of MemPro 1.6.9 on Joomla 3.4.7 w/ Yootheme's Moustache template. I haven't worked on the site for while so I was surprised when I discover the Signup button on the MemPro breaks the template (see attachment). Given what I know, this is usually caused by errand HTML tags (from Signup)... But I don't know where to look. If you need backup login credential, pls let me know if there is a way to submit that to you

I am not sure Membership Pro 1.6.9 can work well with Joomla 3.4.7. You should consider upgrading your site to latest version of Membership Pro (version 2.2.1 for now) if it is possible

If you could not afford to upgrade, then access to joomdonation.com/support-tickets.html , submit a support ticket sending us super admin account of your site. We are going to check to see what's causing the issue and get it sorted for you
