Option to Hide Form Title

I'd like to suggest that you add an option in the form settings page where you can select to hide/show the form title. Seems it would be an easy feature to add.

I saw in the forum how to modify default.php to remove the title, but it would be much simpler if it could be done through a setting in the configuration for each form.
