could you tell me how (which file) to modify the appearence and/or hide the "Total Amount" and "Tax Amount" fields?

Where you want to modify it? On the form? Or in the email? Could you send me the link to the page so that I can tell you the file need to be modified?

Please submit a support ticket sending us super admin account of your site and the link to the form. We are going to check, modify code to hide the field for you
