processing my csv file

Hi,
i have customized a template and prepared a csv file to match. i then tried to process it but have received an error in the uploading. what could have i done wrong to cause this error?

after uploading the CSV file if i click on save it gets saved and it comes up Template successfully saved, and if i click on Process than it comes up a sever error.
what am i doing wrong ?
