It works almost 100% except for the text line right at the top. The "Amount Due" never updates to match the field at the bottom of the form.

That [AMOUNT] tag should only be used in case the form has fixed payment amount. In this case, I think you should remove it. Go to Payment Form -> Configuration, look at Messages tab, find Form Message, change it to remove the text The amount you need to pay is $[AMOUNT] from the message
